Who is Eligible
From infants to age 21 that require some form of skilled care. This can range from medication administration to tube feeding and tracheotomy care, as well as many other medical conditions.
How to Enroll
Children are accepted year around including the summer and extended after school care. Transportation is provided to all eligible children enrolled. Parents may visit the Center for enrollment between the hours of 7:00a.m. to 7:00p.m. Monday through Friday. Arrangements can be made with Administration should you need alternate hours including weekends.Enrollment is available to any child regardless of race, color, religion, sex or national origin. Creative PPEC does not discriminate in enrollment and complies with the Americans with Disabilities Act. Waiting lists become necessary when occupancy are full. Parents will be contacted when another opening becomes available. The following items are required for the enrollment process:
- Prescription from Pediatrician
- Nursing assessment completed at Creative PPEC
- Current health record (Creative PPEC will obtain from Pediatrician)
- Completed enrollment forms (obtained at Creative PPEC)
- Current Immunization Record (Creative PPEC will obtain from Pediatrician)
